Community Living Support (CLS)
Community Living Support (CLS) Services are individualized supports intended to assist individuals with Developmental Disabilities. The services are goal oriented, and assist persons with the achievement, preservation, or improvement in skills. The services are offered to individuals who live in their own or family home. Personal care/assistance is an important component of CLS services, and is specific to the needs of the Individual, as determined by the Supports Intensity Scale, the Health Risk Screening Tool, and other person-centered assessments. The service needs may be addressed in one or more of the following areas: Activities of daily living, personal grooming and health care, interpersonal relationships, mobility, home management, and use of leisure time.
